MANILA, Philippines – The Social Security System (SSS) will roll out new contribution payment forms for its individual payors.
SSS said voluntary, self-employed, overseas Filipino workers, and non-working spouses will be required to use the new forms by September 1.
The forms were consolidated to streamline procedures and to make paying more convenient for its members.
Employers will continue to use the SSS Form R5 in the payment of their employees' contributions until the current stock of the form is consumed.
By November 1, 2015, employers are required to start using the new contribution payment form.
For clarifications on the new consolidated contribution form and schedule of their usage, members can inquire through the 24/5 SSS Call Center at 920-6446 to 55; and www.sss.gov.ph.
